I've implemented b) in ui-refresh.

The size/height is as designed - it was bigger in old versions.

There is still no filter in the breadcrumb bar. The size is definitely increased.

The original issue was filed in 2013, and since then we had a complete redesign, the issues don't apply anymore, hence closing as designed.
