Basic text selection is unreliable. Half the time I copy-paste something, I find upon pasting that the first character is missing. I made this screen-cast to demonstrate:
It has been this way in MonoDevelop always and still is in version 3.0.6 which I use now. I use Mac OS X 10.6.8.
It looks like the buggy behavior is more likely to happen the faster the mouse cursor is moved, which leads me to guess that the start of the text selection is only tracked from the mouse position of the first drag event instead of from the mouse position of the mouse-down event.
Was hard to reproduce it :) - but you were right that the position of the mouse down event wasn't taken.
Today we have checked this issue with following builds:
Mac Lion and Mountain Lion.

Mono - 2.10.11 (mono-2-10/2baeee2)
Now selection is working fine. As when we select text it does not exclude first character. Below is the screencast for the same:
