The iOS toolbar gets re-added on each launch of Visual Studio, despite being removed previously.
We don't really control that aspect from within the extension (even though we'd like to, in fact). The toolbar is defined in the extension metadata and is fully managed by Visual Studio. I'm going to leave this bug open, should it turn out to be our bug, but I doubt it is...
Given that the MFA toolbar (or any toolbar, for that matter) does not exhibit this problem, I'd say it's our problem.

Which version of MTVS is that?
This issue was already fixed at some point. Alex can you provide version information please?
On 1.0.897 (the release build) I get
That's on Windows 7 x64 with VS2012
I am using MTVS Beta 4 with windows 8 and VS 2012

Sorry guys, this bug is still there for me, I thought it was a bad installation again and I just reinstalled my computer twice, Also I was able to replicate this issue on a customers Windows machine too yesterday.

Please keep in mind that what Alex is reporting is not the same as the original bug report. Fixing the original issue will probably fix Alex's issue, but just fixing Alex's issue will not necessarily fix the original issue.
It's related though, at least as far as our code is concerned.
I just wanted to let you know that i am facing the exact same issue as Alex showed here. My Environment is Win7 with German localization, VS2012 with German localization and mtvs-1.0.897.0
I believe the Issue Alex and MBI are having, is related to using a non-English VS. I've installed a German VS here, but still not been able to reproduce yet.
Here is how i can reproduce the issue:
1. Open VS (duplicate Toolbar items have been removed beforehand)
2. Open Xamarin.iOS Solution
=> Two Toolbar Items get added the Standard Toolbar
Remove the obsolete toolbar items:
1. Right click on VS Toolbar, "Anpassen"
2. Choose Tab "Befehle"
3. Choose "Symbolleiste: Standard"
4. Remove the unwanted entries ("Projektmappenplattformen", "Projektmappenkonfigurationen")
5. Close VS
Next time i open the Xamarin.iOS Solution the toolbar items get added again.
If there is anything i can do to assist you just let me know, e.g. review or provide any VS config files?
Today we have checked this issue on following builds :
VS 2012 / 2010
The iOS toolbar is not re-added on each launch of Visual Studio.
Its working fine. Hence changing its status to verified.

With mtvs 188.8.131.52 the problem is still there for me.

there are 2 bugs in this issue. Vinicius fixed one of them, but I've just fixed the duplicate toolbar issue as part of 9eb0d258c04b6fb05c0d5f609f3131d210b399db , which Alex Soto just confirmed that it worked for him.
I hope we can include this in the next public release.
