I finally got it to deploy but unplugging my iPhone 4, rebuilding the app, plugging the phone back in, and then uploading to the device.

Errors while pairing are uncommon. Could another application, e.g. Apple's Instruments, been in use ? e.g. still running and had a (past) connection with the device ? (sadly some apps do no close their connection).

note: unpluging (and re'plugin) the device will kill any remaining connection, allowing MT to do it's own.

Instruments was not running at the time and I wasn't aware of anything else running that might have had a connection other than possibly iTunes.

I did previously have two devices plugged in though, the iPhone 4 and my iPhone 5. However, I had unplugged the iPhone 5 by the time I received this error. 

Sorry I don't have more details to troubleshoot the issue.

This bug has been in the NEEDINFO state with no changes for the last 90 days. Can we put this back into the NEW or CONFIRMED state, or are we still awaiting response?

If there is no change in the status of this bug over the next two weeks, this bug will be marked as NORESPONSE.
