When you remove a file (content) via Finder, it no longer appear "red" in the file list.
Only when you highlight the missing file in MD does it appear. See the before and after highlight screenshots I have attached.

Highlighted now appears red
As of the newest MonoDevelop the problem still exists.
MonoDevelop tracks file changes to some extent, but it can't track all changes done in the file system. If you want to update the tree, you can use the Refresh command.
