Discovered the issue from replacing one line of code to solve a bug in the app. The altered line of code did not fix the bug. Had a breakpoint in that line and every time running the app it seemed to run well and VS stop on the break point as normally, but then realized that the problem was not the line, the problem was that the new code wasn't actually running in the device being tested on.
 
To try to resolve the issue, in VS 

1. cleaned the solution
2. Rebuild the solution
3. Uninstalled the app from the device
4. Ran it in debug and release mode.
5. Rebooting VS seems to have resolve the issue

VS also crashed while trying the above.

Another situation was when a new method was added in a class and started to debug with a breakpoint in the new method. VS didn't stop at the break point until VS was rebooted.

Can chance you could attach the project you were working on. Also a screen cast of the crash would be helpful.

I have been unable to reproduce this bug with the last 4.12.4.20 version (part of XVS 3.0.54). I am going to close this.

In case this bug arises again, please provide further information such a device model, API version, project type and screenshots, if possible.
